The Master Agreement contains the provisions that govern all of the entity's MFM accounts with the Foundation. It also contains contact information between the entity and the Foundation. The Master Agreement needs to be signed by authorized individuals from both the entity and the Foundation.
Investment Options and Allocation Form
The Investment Options and Allocation Form contains a complete list and a brief description of all church funds management options available to investors through the Foundation's Ministry Funds Management program. This form is used by the entity to allocate the funds invested among the various options, including CBL Term Investments, Market Investments, and Cash Funds. This form also allows the entity to list those individuals authorized to discuss the transactions specific to an account. An allocation form is required for each account maintained by the entity.
Existing Customer Allocation Form
This form is for those clients with existing CBL Term Investment funds that would like to make additional investments into CBL Term Investment funds.
Ministry Funds Management Banking Authorization Form
This form authorizes electronic transfers between the client's bank and the Foundation.
Ministry Funds Management Policy and Guidelines
The Ministry Funds Management Policy and Guidelines contains the overall governing policies and provisions of the Ministry Funds Management program of the Foundation.
